This is a common lisp thing I think. CL code "compiles" into a "system image". To build it an base image is started that includes the lisp machinery, and then loads in your code. It then saves the system to disk. When you run the resulting image, your simply reloading the state of the interpreter and asking it to evaluate more code. I think its common in that ecosystem to have systems that update them selves mid-operation.
So I think there are some use-cases where you would want to hot load new code into a system, and then save that system latter.
However there are drawbacks. Your program its self becomes program state, and can become hard to reproduce. That can cause very difficult bugs that appear in the running program but not in the source code.
They are the same. E.g. a class decorator use the class variable (aka attribute) to add new methods to the class to extend it.
